Libraries Employee IIPP Checklist & Core-Emergency Preparedness Online Training

NOTE: Faculty, Staff, and Student Employees will need to use either Chrome, Safari or IE 10 & 11 in order to print the documents.  Do not use Firefox. 

The University's Core-Injury and Illness Prevention Plan (IIPP) and Core-Emergency Preparedness (EP) are designed to make employees more aware of workplace health and safety. These programs are sponsored and maintained by the University's Environmental Health and Safety Office (EHS). The goal of the program is to encourage employees to make safety and health a priority in their daily lives. The success of the Core-Injury and Illness Prevention Plan and Core-Emergency Preparedness depends upon the involvement of all levels of employees and administration.

Since the Injury and Illness Prevention Plan (IIPP) and the Emergency Preparedness (EP) are required by state law, all new employees are required to complete the online training course for both. However, the EHS versions of the IIPP are not required for Library student employees because of EHS approval to have only a checklist.

 

General Workplace Safety Rules:

The following are general workplace safety rules that apply to all University of Nebraska-Lincoln employees.

  1. No employee is authorized to conduct any task that he or she knows is not safe or healthful without implementing the appropriate controls to minimize hazards.

  2. No employee will be reprimanded or discriminated against for reporting unsafe conditions or acts.

  3. All employees are encouraged to share their observations and ideas for enhancing workplace safety with their supervisor, unit safety committee, EHS, and the Chancellor’s University Safety Committee.

  4. All employees share responsibility for maintaining a safe work environment. All potential hazards that are beyond an employee’s authority or expertise to correct or fully evaluate are to be promptly reported to a supervisor or other authority.

  5. Every employee is expected to adhere to all applicable safety procedures and requirements contained in this Plan and other safety/compliance plans developed in response to specific regulatory requirements.

  6. Supervisors are expected to continually monitor employee safety performance and take action to ensure that employees act in a safe and compliant manner. Supervisors are expected to serve as role models in this regard.

  7. Every employee shall ensure that they are familiar with emergency procedures, including designated evacuation routes, alarms, and designated shelter-in-place locations.

  8. Horseplay is prohibited.

  9. Employees are prohibited from working while under the influence of alcohol and/or drugs.

  10. Employees are encouraged to discuss with their supervisors personal conditions/situations that may adversely impact their ability to work in a safe manner (e.g., stress, fatigue, illness, etc.).

  11. Appropriate Personal Protective Equipment shall be worn at all times when conditions warrant and in specific designated locations.

  12. All employees must observe good housekeeping practices. This includes ensuring that stairwells, exit routes, exit doors, and access to electrical panels are free of obstruction. It also includes ensuring that spills, accumulated liquids, or other conditions (curled mats, cords across walkways, accumulated ice, etc.) that present a slip or trip hazard are promptly reported/mitigated; and that any excess paper, cardboard, and other combustible materials are eliminated.

  13. Furnishings must be in good condition. Chairs, file cabinets, shelving, and other items that are not in good condition must be removed from service.

  14. Machine guards and other protective devices (such as interlocks) must be maintained in proper position and working order. Machines or equipment with defective or missing guards or interlocks must be promptly removed from service.

  15. Employees shall refrain from moving heavy objects unless specifically trained in proper techniques and authorized to conduct such tasks. Appropriate mechanical devices shall be used when such use will greatly minimize the risk of injury.

  16. Employees shall not handle or tamper with any electrical equipment, machinery, or air or water lines in a manner not within the scope of their duties.

  17. Employees must promptly report all occupational injuries/illnesses. Employees are encouraged to report “near misses” to their supervisor so that appropriate action can be taken to abate hazards before an injury occurs.

  18. The appropriate and proper tool or equipment shall be used when appropriate. Using a tool or equipment in a manner other than intended is prohibited. Tools and equipment that are not in good condition must be promptly removed from service.

  19. Extension cords are to be used only for temporary applications, and only when in good condition and equipped with a ground prong.

  20. Ladders must be used in accordance with manufacturer’s recommendations and limitations.

 

After completing the EP course, participants must fill out the online registration form which goes to the EHS Office to record successful completion of the training. Both courses should be completed within three weeks of a new hire. If you have questions about the online training courses, please contact Brent Morgan or the EHS Office.

 

IIPP CHECKLIST:

Once the employee has read through the following checklist and action items, the Injury and Illness Prevention Plan (IIPP) portion is complete. Using the “Print Page” link below, the employee needs to print, sign and date the document which is given to the immediate supervisor. The employee may retain a copy for him/herself if so desired.

1) Tour of the library/building(s) in which they will work to indicate fire exits, access to tornado/severe storm shelter or other building-specific security items, such as Shelter in Place (provided by supervisor).

3) Emergency phone numbers (911, campus police (2-2222), supervisor(s)).

4) Information of what to do locally in the following instances:

a) Fire alarm: if a fire alarm sounds, exit through the nearest fire door. If you observe a fire, leave the area, pulling a fire alarm on the way if there is one. If not, call 911 as soon as you reach a place of safety.

b) Tornado/severe weather warning: If you hear tornado sirens or a voice activated alarm, or are directed by a library staff member, move to the nearest tornado shelter or interior room and remain there until given the all clear.

c) Shelter in place procedure: What to do should campus or city police inform us someone with a firearm is in the vicinity, and that we need to secure the building.  Link: https://safety.unl.edu/procedure/shooting-incident 

d) Bomb Threat: No signal has been designated for bomb procedures. In case of an emergency evacuation, follow the procedures for Fire Evacuation. For non-emergency situations, please exit the building promptly through the main exit, as directed by library staff. (More information on this is provided in the Emergency Preparedness section.)

e) If the employee is injured while at work: This information is on the Human Resources website: https://hr.unl.edu/benefits/riskmanagement/workerscomp.shtml

f) If the employee observes someone in distress or a safety issue: If the person is obviously in some sort of medical crisis: unconscious, having convulsions/seizure, bleeding profusely, go to the nearest phone and call 911. Then let your supervisor know that you have called 911 and where the person is located. If someone appears to be intoxicated or not in need of immediate medical attention, contact your supervisor so they can let a circulation staff member know that campus police should be contacted to request a health check.

Please report safety issues and any near-miss/close calls to the supervisor. 

A near miss is an incident where no property was damaged and no personal injury sustained, but where, given a slight shift in time or position, damage and/or injury or illness easily could have occurred. It can be thought of as a "close call." EHS encourages reporting of near-miss/close call incidents so that contributing factors can be identified and abated before they result in personal injury/illness or property damage. By reporting these circumstances you are contributing to a safer and healthier campus environment. We appreciate your participation and assure you that there is no risk of repercussions for reporting a situation or hazard.

Examples might include:  loose or missing carpet tiles, slick/wet floors or stairs, broken windows, chairs, or other equipment, or faulty electrical switches.

*Additionally, employees should have the phone number of their direct supervisor, functional supervisor(s), who to contact if their supervisor is not available, and the department chair or manager of their department/unit.

*Employees are encouraged to sign up for University of Nebraska-Lincoln alert at:  https://unlalert.unl.edu/unlalert/    to receive emergency alerts.

*Supervisors may ask their employees to read additional information.  If a employee's supervisor deems that their employee needs to take the full EHS IIPP, then the employee must do so.

*By signing and dating this document, all newly hired employees are acknowledging that they have received information on the topics outlined in the checklist above.

EMERGENCY PREPAREDNESS (EP)

The Emergency Preparedness (EP) portion is taken through the University of Nebraska-Lincoln Environmental Health & Safety online web-training site.

  • Read Overview and Steps to Take EHS Web-Based Training
  • Scroll down to the Training Menu
  • Click "Core 2 - Emergency Preparedness Training"
  • Click "Core - Emergency Preparedness Training"
  • Enter your NU id information when prompted

2. After achieving an 80% score, you have completed the required training.
